Referee Online Reporting
This form is for filing of FCSC Official Referee Game Cards.
We ask that you (and Assistant Refs) enter as much information as you know. This will help us insure that we pay you for the correct game. If you are one of the assistants you should enter the best guess for all fields. If you have no idea you may enter "Don't Know" in any field.
Verification:
Fields labeled with (*) are required fields. If you don't have info for all the required fields, you must enter "None" or "Don't know."
Note: This report may not be used for CSYSA competitive games. Competitive CSYSA games must be called in directly to CSYSA at 1-800-490-8040.
